FOXO4-DRI: A Senolytic Peptide for Clearing Senescent Cells

FOXO4-DRI falls under a class of compounds called senolytics, which is a fancy way of saying it cleans out senescent cells. Think of senescent cells as zombie cells, old cells that have stopped working right and are clogging up your system. As we age, environmental stress, life stress, and poor health pile up more of these zombies in our tissues.

This peptide goes in and clears them out. You run it once or twice a year, kind of like a tune-up.

What It Actually Is

FOXO4-DRI is a synthetic peptide engineered to eliminate senescent cells. Dr. Peter de Keizer's team first developed it in 2017. In mice, the therapy reversed aging signs like fur loss, improved kidney function, and increased activity levels.

The peptide targets FOXO4, a protein elevated in senescent cells. By disrupting the FOXO4-p53 interaction, it triggers these dysfunctional cells to self-destruct while leaving healthy cells alone.

That selectivity is the key. It kills the zombies and spares the healthy cells.

How It Works

In senescent cells, FOXO4 binds to p53 in the nucleus. That binding prevents p53 from triggering apoptosis (cell death). So the zombie cell just sits there, causing inflammation and damage.

When you inject FOXO4-DRI, the peptide competitively binds to p53 and breaks up the FOXO4-p53 complexes. P53 gets released from its nuclear prison and relocates to the mitochondria. From there it activates caspases, the cell death enzymes, and the senescent cell self-destructs.

Healthy cells barely express FOXO4 at all. So the peptide is essentially invisible to them. That's why studies show apoptosis signals firing in senescent cells while non-senescent cells remain unaffected.

The peptide also uses a retro-inverso structure to resist enzymatic breakdown, plus a cell-penetrating sequence derived from HIV-TAT to cross cell membranes efficiently.

The Benefits

Most of the data we have is from animal models, but the findings are pretty compelling.

Cellular rejuvenation. Older mice showed reversed fur loss, improved kidney function, lower urea and creatinine, and restored organ health. Tissues had fewer senescent cells.

Increased vitality. Treated mice nearly doubled their running distance on exercise wheels.

Tissue regeneration. Reduced senescent chondrocytes by over 50%. Also reduced lung fibrosis.

Reduced inflammation. Lower IL-6 in kidneys and other tissues. Reduced SASP (senescence-associated secretory phenotype) factors.

Improved vascular health. Better endothelial function and vasodilation in aged mice.

Hormonal restoration. This is the one I care about right now. It cleared senescent Leydig cells in the testes and led to higher testosterone levels in aged mice. This is why I'm personally running it for fertility purposes.

To be clear, this is not a replacement for testosterone therapy. But for guys like me who've been on TRT for years, it can help kickstart endogenous production and sperm motility. If you refuse to go on TRT, this could still help.

My Dosing Protocol

In animal studies, they used 5 mg/kg every other day, three injections per week.

For humans, the internet protocol you'll often see is 3 mg subq for 6 days, totaling 9 mg.

I prefer to take that same total and split it differently. I do 1 mg per night before bed, Monday through Friday, for two weeks. That gets you 10 doses and you're done for the year.

It must be injected. There is no oral option. Comes as a lyophilized powder, reconstituted with bacteriostatic water.

You don't want to use it continuously. Short cycles are what works best. The body needs recovery time because it goes into rest and repair mode.

Side Effects and What to Expect

Animal data showed no organ toxicity after nearly a year of treatment. No adverse behavioral changes. No damage to healthy cells.

In humans, side effects are mild. Some flu-like symptoms in the first few days, which both my wife Taylor and I experienced. Minor injection site bruising, less than GHK in my experience. Some people report feeling a little off on injection days because the body is in repair mode.

Here's a rough timeline of what I noticed.

Week 1. Slight cold/flu-like feeling. Stuffy nose that cleared up after.

Weeks 2 to 4. Better recovery, less stiffness, better sleep. My Oura ring confirmed deeper sleep. More energy on waking.

Months 2 to 3. Better strength, less inflammation, clearer cognition. Skin felt softer. I also noticed more hair growing on my back, which is just genetic for me. I'd rather have that than lose hair on my head.

Older users with more senescent cell burden will likely notice bigger changes than I did at 30.

Who Should Run It

Anyone over 60. No-brainer. Once a year minimum.

People with osteoarthritis, pulmonary fibrosis, COPD, chronic kidney disease, or type 2 diabetes with metabolic inflammation. These are conditions where senescent cells drive pathology.

Couples actively trying to conceive. I think both partners can benefit. That's why I'm using it right now.

My Take

FOXO4-DRI is not in my top five peptides, but I think anyone over 30 should run it once a year. One vial runs you two to three hundred bucks and covers your whole cycle.

You won't feel it slap you in the face like Retatrutide or something therapeutic. The benefits are cumulative, similar to SS-31. A few weeks in, you just feel less inflamed and more clear.

If you've got the basics dialed in and you're asking what's next for longevity, this belongs in the bucket. Animal data is strong, safety profile looks clean, and the mechanism is selective. Human trials are still pending, so go in knowing it's experimental. But for a once-a-year tune-up, I think the risk-reward is solid.

Hey everybody, this is Hunter Williams. I hope you're doing amazing wherever you might be in the world. Today's video is going to be about a peptide that is known as FOX04-DRI. So this peptides falls under the class of what we would call senolytics. which is a really fancy word that basically means it cleans out senescent cells. And when we think about senescents cells, that also sounds like kind of an intimidating word. Basically what that means is that they are zombie cells that are old cells are kind clogged up.

Think if you have an old car or an own house and there's parts of the house that causing issues or parts to the car that're causing issue because they're so old and they need to be changed out. As we age, we do tend to get a buildup depending on the amount of environmental stress, depending the life stress depending or end health that causes more of a proliferation of these senescent slash zombie cells. Now what FOX04 does is it actually goes in and cleans up these cells, what does that mean?

It means it's a peptide we can use once or twice per year to go in, and kind of be like a tune-up to get rid of the senescents cells It's not one that we would use, quote unquote, therapeutically to have fat loss or injury healing or anything like that. But what you'll notice is that over time, when you just run maybe one or two cycles per year, you notice things start to click a little bit better. And I would say even kind of similar to how SS 31 has this cumulative effect over a time where you might not feel it right away,

but you will just wake up a few weeks later and notice, hey, wow, I do feel better, less inflamed, more cognitively clear. And so it's a little bit more of a convoluted discussion around FOX04 than it would be with some of these other ones that are really simple, obviously like run a True Tide. You take it, you lose weight, right? Whereas FOXT04 is a bit different. There's few different mechanisms going on, but that's what we're gonna cover today. I've done a video about this in the past, it was really old, probably at least two years old. So I figured it a good time for an update. And also too, just in spirit of my own health and fertility journey, I'm actually using this because there is evidence to show that FOCT04 can help with

basically fertility, sperm motility, a sperm morphology and whatnot. And so I'm actually using it. So I was like, Hey, it's a good time to do an update just because I have my own personal experience to add into this one of recent that I can kind of talk on today. Today we are gonna go over FOx04DRI and this is the senolytic peptide for healthy aging. So let's cover what it is, what does, and how we could potentially use it for research purposes.

Basically, FOXO4 is a synthetic peptide engineer to eliminate senescent zombie cells that contribute to aging and tissue damage over time. It was actually first developed by Dr. Peter DeKeyser's team in 2017 and the therapy dramatically reversed aging signs in mice by restoring fur improving kidney function and increasing activity levels. The peptide targets FOX04, a protein elevated in senescent cells, and by disrupting a critical FOx04-P53 interaction, it triggers these dysfunctional cells

to self-destruct while sparing healthy cells. And more importantly, that's The key, right, is while sparing the healthy cells while actually killing the zombie cells. So this target approach positions FOXO4 DRI as a potential breakthrough in anti-aging research. One big thing that's out there right now is Clotho and Clotho, the quest for Cloto is kind of like the question of the Holy Grail. Is it the Miracle Anti-Aging Compound that will solve everything? I don't know. Maybe it is, maybe it isn't. I will say in most places, it can be kind of expensive to buy. So I'm not telling people not to use it.

And I think it could be beneficial, obviously, for kidney health and obviously for a host of other things. But I would say FOX04 is a little bit more expensive than some of the average peptides we have out there, but something that I could get over time have a relatively similar benefit when we look at the anti-aging properties. The issue is not a lot of people know about it, not only people not to use it and dose it. And as far as I know, there's not lot people out there talking about hopefully, you know the theme of this year that I've been trying to talk about is bringing some more of these really awesome hidden peptides to the spotlight.

Let's look at how FOX04 works from mechanistic perspective. One, the first thing it does is it blocks P53. and in senescent cells, it binds to P53 in the nucleus of these senescents cells and then prevents the tumor suppression or tumor suppressor protein from triggering apoptosis cell death. Then what FOX04 does is it disrupts the bond. The peptide competitively binds to P53 and it breaks up the FOx04-P53 complexes and releases P 53 from its nuclear prison.

From there, P-53 relocates to the mitochondria when it's freed up, triggering the intrinsic apoptosis pathway and activating cell death. enzymes known as caspases. And then we have the senescent cells, they basically self-destruct. So the Senescence cells undergo programmed death while healthy cells remain unaffected due to low FOX04 expression. The best way to sum that up is that the SENESCENT zombie cells express FOx04 When we inject Fox04, it's going to go bind to this P53 in the nucleus,

bring it out into the mitochondria and then say, Hey, we're going self-destruct. So it was kind of like a computer code to say. Hey you're a bad actor and basically put for lack of a better way to put it, put out and say Hey we are going program cell death because you are hurting the ecosystem of the body to stay healthy. And so that's kind how it works when you inject it. Now let's look at why healthy cells are spared. Basically, FOX04's selectivity stems from a key biological difference.

FOx04 is barely expressed in non-senescent healthy cell. So healthy dividing cells don't accumulate high levels of FOxx04 binding to P53, making them invisible to the peptide's effects. The peptides design as a de-retro inverso structure makes it highly resistant to enzymatic breakdown, extending its potency in the body, and a cell-penetrating sequence derived from HIV-TAT helps it cross cell membranes efficiently. Studies confirm that the peptide activates classic apoptosis signals, which are BAX and BAK, in capsase III, specifically in senescent cells,

while non-senescence cells remain largely unaffected. And FOX04 DRI targets the disorder-transactivation domain of P53 basically providing a molecular key that unlocks the self-destruct mechanism only in cells where FOXO4 has locked it away. And so again, all that to sum up, it basically is selectively going to the cells that potentially are causing harm to and self-destructing and clearing them out. Now we're going to see kind of what are the benefits of doing that.

The first one is cellular rejuvenation. When we use this compound, older mice have shown reversed fur loss. You would have reversed hair loss? I don't know. Maybe, maybe not. It's not anything I've heard people overwhelming report, but it's also not a peptide that people have really used. I'd be interested to see how that translates to humans. We've seen improved kidney function, lower urea and creatinine, and restored organ health. Tissues exhibited fewer senescent cells and more healthy cells in these mice. we also see increased vitality.

Treated mice nearly doubled their running distance on exercise wheels and showed higher activity levels. accelerated aging mice became noticeably more active and exploratory, which is pretty cool. They did notice improved tissue regeneration, so it reduced senescent-burned and cartilage cells by over 50% creating cleaner cell populations for tissue engineering. It also reduced lung fibrosis and improved healing environments for the lungs. Obviously it reduced inflammation, so lower inflammatory cytokines, namely interleukin-6 in the kidneys and other tissues.

It reduced SASP, which is senescence associated secretory phenotype factors, creating more anti-inflammatory environment in body. We also see improved vascular health, improved endothelial function and vasodilation in aged mice. Better blood vessel health may translate to improved circulation and reduce cardiovascular disease risk. And then interestingly, and this is why I'm using it for fertility purposes, all comes full circle now, it helps with hormonal restoration. So cleared senescent latex cells and the testes leading to higher testosterone levels in aged mice.

It also helps, with potential to mitigate age-related hormonally declines. Now, does that mean as a replacement for testosterone therapy? No. However, for instance, in my case, where I've been on testosterone therapy for years, and my sperm motility because of that, because I was not always using HCG, was slightly impaired from where it would have been. This is something that I can use to kickstart that. So does that mean this replacement? No. However if you're one of those people that for whatever reason refuses to go on to testosterone and therapy, I think at the very least it's gonna help.

I also think if it someone that has been testosterone for therapy years there's still numerous benefits to this. And even increasing our own endogenous production alongside the exogenous testosterone use is something that I think is beneficial. When we look at beyond just basic rejuvenation, this again, not to go down a rabbit hole, but cancer therapy support. It may reduce therapy-induced senescence and side effects, radiosensitize tumors and reduce pulmonary fibrosis and lung cancer models have been observed. Then also we see fibrocyst reduction, targeted senicent fibriblasts and keloid scars and long tissue.

And that resulted in reduced pathological scar formation and organ fibiosis, which is pretty cool. So looking at some of the evidence from studies, we had a 2017 foundational study demonstrated FOXO4 reversed aging in mice, improved their fur, activity, kidney function, and 10-month treatment showed no toxicity. In 2020, this is where we noticed the hormonal health. So FOxO 4 cleared senescent latex cells, restored testosterone in the aged male mice. And it's a therapeutic potential for late onset hypogonadism.

2021, there was a tissue engineering study that showed that it killed 50% of senescent chondrocytes, reduced SASP in cartilage tissue and the potential for osteoarthritis cell therapies arose. 2021 we also saw cancer and fibrosis, radiosensitized lung tumors, reduced radiation-induced fibrosis and improved cancer treatment outcomes. And then 2025, it improved endothelial function and vasodilation in aged mice. In 2025 we also saw keloid treatment, induced apoptosis and keload fibroblast, which is introduced as a novel therapy for pathological scarring.

All that to say, those are all animal trials, in case you didn't pick that up. So no human clinical trials published yet. There is a company called Clara Biotech developing fourth generation peptide with enhanced binding, moving toward phase one trials in humans. And so maybe in the next three to five years, we see that. But when we look at some of this animal evidence, it is pretty compelling to, say the least, especially given that we'd see no toxicity with it. Just to highlight some of the benefits of animal models, nearly one year of treatment in mice showed no organ toxicity, no adverse behavioral changes or

damage to healthy cells, which is really important. Especially when we talk about this, we don't want to be causing any harm to damaged cells. Treated mice maintain normal body weight and blood markers. and it did not sensitize normal cells to DNA damage or chemotherapy, confirming its selectivity for senescent cells only. So the consistent theme across all studies was that FOX04 is a potent senolytic with multifaceted health benefits and remarkable safety and model systems. And then here we see in activity levels, there was a 95% improvement in these aged mice.

Kidney function, we saw around 70% improve in kidney function. 50% clearing of senescent cells, and then 60% reduced inflammation in this animal models, which is pretty cool. Now everybody's part, okay, give me the good stuff. What's the dosing? In animal studies, five milligrams per kilogram IV or IP every other day. The effective regimen was three injections per week for multiple weeks and long-term treatment at 10 months showed no toxicity. Now, where does that lead us for human dosage?

I have my own dose. There's some stuff written out there on the internet that you would do three milligrams of a subq injection every day for six days, which would be a total of three doses. And so you'd have a nine milligram total dose, I like to take that and say, hey, let's do one milligram a day for 10 days. And so I would just do Monday through Friday, one milligrams at night before bed, and then you could do that the next week. So boom, you have 10 doses and you're good for the year. I think depending on your age, if you were 60s or 70s, I'm in my 30s. Once a year will be fine for me, especially for fertility.

In most cases, it all has to be injected. There is no known oral administration. Obviously it comes as a lie-off, lies powder, reconstituted with bacteriostatic water. And with this, we don't want to use it continuously. Again, a short approach is what seems to best now and seems have the most effective time. This also allows recovery over time because what you'll notice is that you might get a little tired from using it because the body's kind of in this rest and repair mode. Looking at some of the safety profile and side effects, animal safety data, nearly one year of treatment showed no organ toxicity,

no behavioral changes or damage to healthy cells. No adverse changes in body weight or blood markers on those mice. Potential impaired wound healing if given during injury recovery for humans just based on the mechanism. Again, I wanted to play this out so like when we look at mechanisms, okay, like potentially we could have some contraindication there, right? But temporary inflammation as dead cells are cleared, injection site reactions are possible. I would say for me, this one does kind of leave a little bit of a bruise. It's not bad. Not even as bad as GHK.

When you inject it sub-Q, you do notice that probably for a few hours after that, that it is kind sore right there. Self-experimenters report only positive outcomes, no serious adverse effects. There are minor reports, and I can attest to this, of feeling off on injection days, but nothing severe. And again, I think it's because the body is in this rest and repair mode. The built-in selectivity for senescent cells means minimal accumulation in normal tissues and the targeted mechanism reduces risk of broad side effects." Now, what could you expect from treatment? I built this more or less off my own experiences and experiments with it.

Week one, you're probably not going to notice any positive changes. I think if anything, people tend to get a little bit of kind of like a cold or flu-like symptoms that definitely happen to both myself and Taylor. But then as you progress through the use of it, you notice, so like I used it for five days for two weeks in a row, and then I was done. In weeks two to four, You do notice better recovery, less stiffness and usually better sleep. And then months two or three, noticeable strength, organ function and reduced inflammation.

So what most people report is a chronic pain reduction, a lot of times in the back neck and Wrist, where a lot of people have arthritis. Improved sleep quality and morning energy. I definitely say that was one thing that big for me is I had a lotta energy when I woke up in the morning and my sleep was deeper, even according to my aura ring. Better respiratory health and clear sinuses. The first week that I took it, I actually kinda had about a stuffy nose and then after that, cleared up any sinus clogging. enhanced concentration and reduced mental fatigue. I definitely noticed that increased exercise, stamina and recovery and possible skin tone and hair quality improvements.

Definitely would say for me, I am not the best person to ask about skincare by any means at all, but I did notice I was like, Oh, kind of like my skin felt softer and smoother on my face. And for me, hair, I mean, i've got a lot of hair. The thing that's funny is like I have tons of air, obviously on head, but for someone like me that just genetically has a whole lot hair that also means I get like hair on back. So I probably noticed like more hair growing on the back from using it, which is just comes with the territory of having more here. But I'd rather have that than, you know, be losing the hair in my head.

Hopefully that happen in the future. Maybe it does, maybe it doesn't, who cares. The effects likely correlate with senescent cell burden. Older individuals with more age-related issues may notice more dramatic improvements in younger, healthier users. I think this is kind of the same thing, even if you look at similar things that we would maybe categorize as senolytics, like a clotho. Someone in their 50s or 60s will probably notice much more drastic improvements than someone in the 30s, just because they obviously have 20 or 30 years of more accumulation of these senescents cells. And so for someone their fifties or sixties, this could be...

Even for me, it was a really good thing to take, but I It could be even better. So we look at ideal candidates. I think for people over 60, this would be a no-brainer for me to use once a year. Think everyone can do it. It's just they're going to experience a lot better results. Those experiencing significant age-related decline like frailty, chronic fatigue, organ function issues, and have a higher senescent cell burden means there's going be greater potential benefit for them. I definitely think for specific disease populations, people with osteoarthritis could be big, pulmonary fibrosis would be huge, COPD would huge.

Chronic kidney disease, it would a huge type two diabetes with metabolic inflammation, conditions where senescent cells drive pathology, which are a lot of those chronic inflammatory things. Obviously biohackers, longevity enthusiasts in their 40s and 50s showing aging signs and are comfortable with self-injection and their experimental nature. Again, that's one of the things, we just don't really have a lotta data on it. And so even though the mice studies showed no organ toxicity, You know, we don't know because it hasn't been studied at a large scale in humans. And then lastly, for someone like me, fertility and one cycle for couples, I think the man and the woman should do it both actively trying to conceive

and it would be beneficial. So just some key takeaways. Remember it's a targeted mechanism. It disrupts the FOX04-P53 interaction to selectively eliminate senescent cells while sparing healthy tissue. It's been proven very effective in animals. Human trials are pending. Hopefully we see some of those coming down the pipe. I think they'll probably be able to develop a patented version, thus being able bring to market something. You know, the average cost to bring a drug to the market is like $2 billion. And then it's obviously still experimental, so just be aware of that.

Although I have nothing to worry about and have used it myself with no negative side effects other than maybe that kind of like flu-like symptoms right off the bat when you first start using it. So FOX04 exemplifies precision medicine and aging. It targets specific cellular mechanisms. rather than broad interventions. And as research progresses, hopefully it becomes a cornerstone therapy for extending health span and treating age-related diseases. Maybe not as a frontline treatment, but something that everyone does once or twice per year and receives good benefit from.

And that is it for the slides. And, that, is my summary of Fox04 DRI. This is definitely, I wouldn't say it's like, you know, my top five peptides, but I think it is something that after the age of 30, everyone should run once a year. You know it a little bit more expensive, usually you'll see this for like two to three hundred bucks. but that one vial is going to be your one cycle. And so you're just going use it once per year, and then you were going notice the benefits over time. It's something that I would just kind of do, you know, everyone's always asking for longevity protocol. This definitely falls in that bucket. You know obviously it's a scene-alytic, it falls into that longevity of hey, everything is 95% great for me for the most part,

but what can I do to really be on the cutting edge, to take it to the next level? And I'd put Fox04 for sure in the bucket, so hopefully this was helpful to you guys. Again, I think it is one that Pretty much everyone out there in our ecosystem, if you're listening to this, I think there's a use case for, whether it's more on the chronic disease side of things, just to be an adjunct to some of the other therapies we talk about, or if it is just for longevity and doing your best to maximize and live your life, right?
