The Shocking Anti-Aging Power of Cialis Beyond Erectile Dysfunction
Last year I made a video about Cialis for anti-aging and it became the most viewed video on my channel. Almost 100,000 views. Funny enough, the one time I just spouted off about Cialis, it blew up bigger than anything else I've covered. So today I'm going much deeper, with new research, and I'm adding something most people never think about. Cialis for women.
What Cialis Actually Does
Cialis, brand name for tadalafil, is a PDE5 inhibitor. It blocks the PDE5 enzyme, which normally breaks down cGMP. cGMP is the molecule that mediates nitric oxide signaling in blood vessels.
Block PDE5, and you get more nitric oxide signaling. Blood vessels relax and dilate. Better blood flow.
That's why it helps men get erections. But PDE5 isn't only in penile tissue. It lives in blood vessels throughout the heart, lungs, brain, and muscles. What Cialis does in the penis, it's also doing in the rest of the body.
Many ailments of aging involve impaired nitric oxide signaling and endothelial dysfunction. Cialis hits a fundamental aspect of aging at the source.
Cardiovascular Health
This is arguably the biggest reason to use Cialis.
A clinical trial showed that just four weeks of alternate-day tadalafil more than doubled brachial artery dilation, from around 4% to 9%. The improvement persisted two weeks after stopping the drug. Another study showed one year of daily low-dose Cialis slowed the progression of arteriosclerosis in older men.
Now the big one. A 2024 study in the American Journal of Medicine tracked over 500,000 middle-aged men with ED for around three years. Tadalafil users had:
- 27% reduction in heart attacks
- 34% reduction in strokes
- 34% lower overall mortality
Another insurance database analysis found men on tadalafil had 44% lower overall mortality than men not on ED medication. These are massive differences across hundreds of thousands of people.
Cardiologists call ED a canary in the coal mine for systemic vascular disease. Treating ED with Cialis treats the underlying poor blood vessel function that gets everyone in the end.
Sexual Function and Hormones
Around 50% of 50-year-old men have some form of ED. 60% of 60-year-olds. ED is linked with depression, low self-esteem, and relationship strain.
A daily 5mg dose is FDA approved for continuous use, so you don't have to time a pill before sex. That alone is a big edge over Viagra.
Here's the part most guys miss. Cialis can give you a small testosterone boost. In one trial, 40 middle-aged men took 5mg daily for three months. Total testosterone rose from around 360 ng/dL to 520 ng/dL. Their LH dropped, suggesting their body sensed the higher testosterone and pulled back its own signal.
Is going from 360 to 520 enough to replace TRT? Not really. But it's better than nothing, and for guys who refuse testosterone therapy, it's something. The likely mechanism is improved blood flow to the testes and reduced oxidative stress on Leydig cells.
BPH and Nighttime Bathroom Trips
Daily Cialis relaxes the smooth muscle in the prostate and bladder neck. Trials show 5mg daily significantly reduces BPH symptoms like frequent urination, urgency, and nighttime wakings, with efficacy comparable to alpha blockers.

Cialis for Women
Yes, women can absolutely use Cialis.
Female arousal also relies on nitric oxide and blood vessel dilation. In a landmark study, 33 premenopausal women with type 1 diabetes took 5mg daily of tadalafil for 12 weeks. Results were strong. Arousal improved, orgasms were stronger, sexual enjoyment increased, pain during sex decreased. Objective measures showed increased clitoral blood flow.
Dr. James Simon and other gynecologists have started prescribing low-dose Cialis for post-menopausal women with arousal issues, often alongside hormone therapy. He notes the sexual benefits for women can be as significant as those for men.
For women, I'd start at 2.5mg and titrate from there. Side effects are the same as men. Headache, flushing, nasal congestion. The cardiovascular and anti-inflammatory benefits should apply to women too, especially after menopause when estrogen's protective effect drops off.
If you're a woman not on hormone replacement, please get on hormone replacement. And consider Cialis on top of it.
Brain Health and Cognition
That same study of 500,000 men showed a 32% lower rate of dementia in tadalafil users. Viagra users had 25% fewer dementia cases. The 2021 Cleveland Clinic report on Viagra and Alzheimer's got widespread attention for the same reason.
The theory is improved cerebral blood flow, reduced small vessel disease, and direct effects on neural cells. PDE5 is expressed in the brain, including the hippocampus.
A 2019 randomized study of men with BPH and ED found six months of daily Cialis improved attention, processing speed, and visuospatial abilities. The authors credited reduced systemic and prostatic inflammation along with improved erectile function.
What's good for the heart is good for the brain.
Muscle and Performance
This one excites me as a training guy.
A University of Texas study found Viagra approximately doubled the rate of skeletal muscle protein synthesis in the fasting state. The increase was comparable to a moderate testosterone injection of 100-200mg per week. No steroid side effects. Men on Viagra also performed more reps before fatigue.
A 2017 six-month trial in middle-aged men with mild ED showed daily tadalafil increased lean mass and reduced waist circumference compared to placebo. Other studies show improved insulin sensitivity in men on daily Cialis.
I personally take Cialis before training sessions. Not every time, but often. The pumps are noticeably better, recovery is faster, and soreness drops. Bodybuilders have been using low-dose Cialis as a pre-workout for years. Unlike caffeine-heavy pre-workouts that constrict blood vessels, Cialis opens them.
Inflammation and Endothelial Function
Cialis has direct anti-inflammatory effects. One study showed tadalafil uniquely lowered the release of inflammatory factors from endothelial cells under stress.
A 2019 study on men with BPH found inflammatory cytokines dropped after six months on Cialis. Researchers concluded chronic tadalafil treatment reduces systemic inflammation.
There's also early research on Cialis affecting senescent cells, the zombie cells that secrete inflammatory factors and drive aging. PDE5 inhibitors appear to have anti-proliferative and anti-inflammatory effects at the cellular level. Still theoretical, but interesting.
Dosing
Keep it simple.
Starting dose: 2.5mg daily. You'll get improved nitric oxide, decreased arterial stiffness, energy and arousal boost. Less than 5% of people get a headache.
Longevity dose: 5mg daily. This is what I recommend for most men over 45, women on HRT, anyone with BPH or metabolic syndrome.
Performance dose: 20mg as needed. For sexual performance or harder training sessions.
I run a blast-and-cruise approach, similar to testosterone. Stay on a low daily dose of 2.5 to 5mg. When you want enhanced performance, bump to 20mg. Keeping the daily dose low means you don't desensitize to the bigger dose when you actually want it.
I'll cycle off about one week out of every four to six weeks. Not strictly necessary, but I like the break.
If you've never taken Cialis and you start at 20mg, it's going to be too much. Headache, flushing, bad time. Start low.
My Take
Cialis is one of the most underrated anti-aging medications on the planet. It supports cardiovascular health, preserves sexual function, helps the brain, builds muscle, reduces inflammation, and improves body composition. It's cheap, easy to access, and the side effect profile over long-term use is excellent.
The performance benefits alone are enough for me. The anti-aging and longevity benefits are a bonus. And the fact that women can use it and most have never even considered it is something I want to change.
If you're not on hormone replacement therapy, get on it. And then add Cialis on top. You won't regret it.

which is kind of funny to me because to be is just, I wouldn't say elementary because I'm going to go really deep in the presentation today, but compared to some of the other things that talk about that, and I think deserve more of a spotlight, But I understand it. every single one of the questions that get asked So without further ado, I'm gonna share my screen today. And we're gonna talk about Cialis as an anti-aging medication All right. I am Hunter Williams and today's video is all about ciali's otherwise known as Tadalaphil a premier and I mean premier in every sense of the word, anti-aging compounds. So let's look at Cialis as more than just an erectile dysfunction medication. You've probably heard of Cialsis, the brand name for a drug called didalafil as a well-known treatment for erectiles dysfunction and benign prostate issues.
However, there is mounting large scientific data to suggest that this erectil dysfunctional medication might have broad antiaging benefits affecting the heart, brain, muscles, hormones, and even more. That's what we're going to look at today and go into what other benefits we stand to gain from Cialis, because I think there are many. So just as a brief aerial 30,000 foot view, what is Cialsis? It is a PDE5 inhibitor, meaning it blocks the PDe5 enzyme. The enzyme's job, PD5 that is, is to break down CGMP, which is the molecule that mediates the effects of nitric oxide in blood vessels.
And so by inhibiting PD 5, Cielis enhances nitrous oxide signaling, causing blood vessels to relax and dilate. So the immediate result is improved blood flow. And the reason that helps men get erections is because of the bloodflow. But PDE-5 isn't only in penile tissue, it's found in blood vessel throughout the body, including the heart, lungs, brain, and muscles. What it is doing in the penis, is also doing it in rest of body. An enhanced nitric oxide slash CGMP signaling in these tissues can lead to lower blood pressure, better circulation and improved organ function.
So scientists have discovered that many ailments of aging from hypertension to heart disease to cognitive decline involve impaired nitric oxide signaling and blood vessel dysfunction. Therefore, Cialis's mechanism addresses a fundamental aspect of ageing which is endothelial dysfunction, which usually is what causes the damage that leads to hear disease and dementia and numerous other things in the body. So by restoring healthier circulation and related pathways, Cialis may combat several age-related conditions at once. So let's look at cardiovascular health first.
I think this is arguably, and rightfully so, one of the biggest reasons to use Cialsis. Let's see what it does from improved blood flow and endothelial function. When we boost nitric oxide, cialus helps blood vessels function like those of a younger person. So a clinical trial found that just four weeks of alternate days to Dalafil significantly improved brachial artery dilation, which is a measure of endothelial function, from around 4% baseline to around 9% base line. So it basically doubled it, more than doubling the vessel's ability to relax.
This improvement persisted even two weeks after stopping the drug, Which is pretty interesting because it does have a shorter half-life, but the improvement helped long-term even 2 weeks later. And better endothelial function obviously translates to lower blood pressure and less stress on the heart. Another study, one year of daily low dosialis and men with prostate issues appear to slow the progression of arteriosclerosis, especially in older patients who had high cardiovascular risk. So it definitely helps with arteries and keeping them plastic. Now, when we look at rates of heart attack, stroke, and death.
Perhaps the most eye-opening evidence comes from large population studies. 2024 studies, so very recent, in American Journal of Medicine, tracked over 500,000 middle-aged men with erectile dysfunction for around three years. So those who took PD5 inhibitors, mainly Cialis or Viagra, had dramatically lower incidence of major cardiovascular events compared to those that didn't. Tadalafel users also saw a 27% reduction in heart attacks and a 34% in strokes, and their overall risk of dying was 34 percent lower than non-users.
Niagara users benefit, though, to a slightly lesser degree, around 17% fewer heart attack as opposed to 27 percent reduction. Another insurance database analysis similarly found men on Tadalafel had a 44% lower overall mortality than those not on ED medication. So these are huge differences in that we're talking like 500,000 people. That's a huge difference that persisted even after adjusting for health status and factors. It suggests the analysis is doing more than just treating erectile dysfunction, it is overall improving cardiovascular health.
When we look at some of the mechanisms, cardiologists note that erectile dysfunction can be an early warning sign of systemic vascular disease, kind of like a da da, but it is a canary in the coal mine type lesson when we can look erectal dysfunction. So by treating ED with Cialis, we're also treating underlying poor blood vessel function, which is what gets everyone. Like I said, Researchers believe Cialis lowers high blood pressure, improves blood flow to the heart, muscle, and has anti-inflammatory effects on blood vessels. So all of these would reduce the risk of plaque buildup, clots and vessel blockages that cause heart attacks and strokes.
And there's even evidence Cials can help prevent heart failure and help with heart-failure patients. In clinical studies, Ccialis was used to improve exercise capacity, it lowered pulmonary artery pressure and increased cardiac output in patients that had heart failures. And unlike some medications, it does not adversely affect exercise tolerance or provoke arrhythmias in heart patients. In fact, improving erectile function meant with heart failure was linked to better quality of life and less depression, which we're going to talk about a little bit later. Let's look at heart disease. So aging arteries tend to stiffen and accumulate plaque.
In addition to the blood flow improvements mentioned, Cialis may also directly combat atherosclerosis development. So animal and cellular studies indicate PDE5 inhibition has anti-proliferative effects on vascular smooth muscle and reduces oxidative stress into endothelium. The human study of men with high cardiovascular risk showed that a daily to dalafil prevented any significant worsening of arterial stiffness over one year. And so by keeping arteries flexible and clear, Cialis could help other adults maintain younger circulation.
So obviously very powerful in the realm of cardiac health. Now let's move to sexual function, but let us look at more sexual functions in the context of hormones and quality of life. So when we looked at Tadalafel, obviously restoring erectile function of men, which it's intended to do, erectiles dysfunction becomes increasingly common with age. Around 50% of 50-year-old men have some form of ED and 60% percent of six- year-olds have It makes you wonder, do 100% of 100-year-old have ED? Probably not, probably doesn't scale that way. But ED not only impacts intimacy, but is also linked with depression, low self-esteem, and relationship strain in older men.
And Cialis has revolutionized this treatment. So a low daily dose, which is five milligrams of Cialsis, is FDA approved for continuous use, meaning an older gentleman can maintain sexual spontaneity without having to time a pill before intercourse, Which again, I think is another benefit of cialise over Viagra. And the media benefit has improved sexual satisfaction and confidence, and over the long term, active sexual life is associated with better mood and even longevity. Some studies suggest that regular sexual activity correlates with lower mortality, possibly due to hormonal and stress reduction effects.
And so by treating ED, not for the sake of ED itself, but Cialis may indirectly contribute to mental and emotional well-being as men age. Indeed, experts note that enabling sexual activities can boost mental health and relationship happiness. Let's look at hormonal effects. So ironically enough, there's a little bit of a testosterone boost from Cialis. Now, is it enough to replace testosterone? Of course not. However, it is pretty cool because there was a lot of men out there that would just refuse to take testosterone until the day that I, hey, more power to them.
I don't recommend it, but let's talk about this. In one trial, 40 middle-aged men took five milligrams daily of Cialsis for three months. Their total testosterone rose from 3.6 Nanogram per milliliter, aka 360 nanograms per decilator, to 5.2 or 520. So it gave them a 200 point bump in total testosterone. Now, in my opinion, is 36520 going to do anything for you? Eh, not really, but it's better than nothing for sure. In this case, their luteinizing hormone went down, suggesting their body sensed the higher testosterone and reduced the signal produced more.
Another analysis reported that long-term Cialis use shifts the testosterone to estrogen ratio in favor of testosterone. Now, if you know me, you'll know that I don't necessarily think that we should ever suppress estrogen, but a lot of guys will express more estrogen from the testosterone, whether they're injecting it or they are just metabolizing their own natural testosterone because they were too fat. And when you're too, fat you over express estrogen, from testosterone your body is getting because you have more aromatase enzyme that express them.
So why would Cialis affect hormones? So one theory is improve blood flow to the testes and reduce oxidative stress may enhance latex cell function. I really think that's it. And also by improving sleep and reducing nighttime bathroom trips through the prostate benefits, we're going to talk about Cials could indirectly boost testosterone since poor sleep is obviously very correlative and linked with low testosterone. So it's not a suitable substitute for testosterone replacement therapy and men that need it. However, it could potentially help aid muscle mass, energy, and libido, which are all crucial as we age.
Now, this is the kind of more common benefit that we associate with Cialis, BPH, or benign prostate hyperplasia. So daily Cialsis helps relax the smooth muscle in the prostate and bladder neck via the nitric oxide CGMP pathway. which improves urine flow. So clinical trials have shown five in the grins, daily Cialis can significantly reduce BPH symptoms like frequent urination, urgency and nighttime wakings with efficacy compared to standard BPh medications like alpha blockers.
This means an older man can take one medication to simultaneously treat ED and ease his Bph, improving sleep and comfort and better sleep itself is obviously going to help with aging. and then we can avoid BPH complications like urinary retention or recurrent UTIs. So BPh itself is inflammatory in nature and tied to metabolic syndrome. By reducing prostate inflammation, Cialis addresses aspects of a man's health that is both a disease and an aging marker. A lot of men notice that if they take Cialis, their nighttime bathroom trips are dramatically reduced.
I know it's not everything. And if you get Libidon or you can get the prostate stack of my regulators with Cials, the Cielis helps the Prostate stack work that much better to help with BPH and just helping with nighttime, bathroom, trips. Now I did say that I was going to talk about women and I'll talk a little bit about this more. But when we look at women and Cialis, women can absolutely use Cialsis. But yes, woman can benefit from Cielis so it's not officially approved for female sexual dysfunction. Research and clinical practice shows promise, especially for women with arousal difficulties.
So the physiology of female arouse, if that'd be a good book title, also relies on nitric oxide and blood vessel dilation, the same way that erections in men relies it. And in a landmark study, 33 premenopausal women with type 1 diabetes, who often have impaired genital blood flow, took daily to that level of five milligrams for 12 weeks. And the results were pretty compelling. Arousal greatly improved, orgasms were stronger, sexual enjoyment and frequency increased, and even pain during sex decreased. So objective measures showed increased clitoral blood velocity and a better vaginal blood-flow pattern.
And go figure, the women also reported improved overall quality of life. Although this was an open-label study with no placebo group, it provides proof of concept that boosting general blood flow helps women's sexual function. There's also anecdotal evidence from gynecologists and sex experts that have begun prescribing low-doseyalis or related drugs for post-menopausal women with arousal issues, Often in combination with hormone therapy, Dr. James Simon is one of these and he notes that the sexual benefit for women, improved arousal and stronger organisms can be as significant for them as the
benefits men experience. So if you're a woman, you can definitely benefit. Just as a note for woman it's generally safe. In fact, the higher dose is an approved treatment for pulmonary arterial hypertension in both sexes indicating that regulatory agencies like the FDA consider it risk benefit profile acceptable for a women. Common side effects that women, if you've not taken, may experience are headache, flushing, or nasal congestion. Every guy that's taken too much knows that. I know I sure as heck do. But one area of interest is whether women could gain the same cardiovascular protections that men do from long-term cellulose use.
There's reason to believe so. Cardiovascular disease in women share similar mechanisms, especially after menopause when estrogen's protective effect wanes, which is why every woman should be on hormone replacement after using or after every woman should be on hormone replacement after they go through menopause because they're not going to have any hormones. So there are still formal trials needed in women, but so far there's no biological red flag that makes it unsafe for women. It's just typically thought of for men and was developed before men. Now moving along to cognitive health, brain aging and mood.
This is where it's pretty cool because most people Don't really ever think about this when we think of these neurodegenerative disease. But like I talked about in that study of 500,000 plus men, those until the Dallifil had a 32% lower rate of developing dementia compared to men not on PD-5 inhibitors. Viagra users also had 25% fewer dementia case than non-users. And while these findings are observational, the size of the effect is impressive. I mean, 32 percent lower rates of dementia is profound compared than men that were not using them.
There's also a 2021 report from the Cleveland Clinic. We got widespread attention in a retrospective analysis of insurance data of men taking Viagra had substantially lower Alzheimer's disease rates than non-users, leading the authors to propose Viacra for Alzheimer prevention trials. So how does it protect the brain? The theory is one, improve cerebral blood flow, reduce small vessel disease and possible direct effect on neural cells. And I think when we look at neurodegenerative disease, this is what is playing a huge role in people developing Alzheimer and dementia is the inability
of them to circulate blood into the So PD-5 is expressed in the brain, including areas like the hippocampus, albeit at lower levels than in peripheral tissue, and so enhancing CGMP signaling in brain may promote better neuron function and memory formation. So what's good for the heart often benefits the So let's look around some of this. There was something called the PASTIS trial, which was in older adults over 70 with cerebral small vessel disease, and they were given to Dallas OTC if it boosts brain blood flow. The trial reported some improvement in cerebral perfusion and treated individuals suggesting that PD-5 inhibition can indeed increase blood supply to aging
brain tissue. There's also the dream study investigated if PD-5 inhibitors could slow Alzheimer's progression and found no conclusive evidence of benefit. However, it's very complex topic. And so we don't have a smoking gun proof, but at the very least it is not going to hurt. Next, a 2019 randomized study of men with benign prostate enlargement and erectile dysfunction, where the average age was around 60, found that six months of daily Cialis led to better performance on multiple neuropsychological tests.
So these men initially had lower cognitive scores, probably due to the chronic discomfort and inflammation. from their condition, but after treatment, their attention, processing speed, and visuospatial abilities improved to match healthy controls. The same study also measured an electrophysiological marker called mismatch negativity, which relates to automatic auditory attention and found Cialis improved the brain's response as well, indicating enhanced cognitive processing. And the authors also attribute the cognitive gains partly to reducing systemic and prostatic inflammation with Cialis as well as improving erectile function.
So it definitely helps with mental health. Speaking of mental health, by effectively treating erectile dysfunction, Cialis can alleviate the psychological distress associated with sexual failure, which is not so much physical in nature, but is very real as it relates to our physical health. So in men with ED, successful treatment has been shown to reduce symptoms of depression and anxiety and improve self-esteem. There's a 2024 study and it found that there was an ability to engage in sexual activity may contribute positively to mental health for patients.
And so beyond the indirect effect, Cialis could have an antidepressant or mood effect. There was some preclinical evidence showing that CGMP signaling influences neurotransmitters related to mood. So it can affect serotonin and dopamine pathways. A few small trials have looked at PD-5 inhibitors and conditions like major depression with hints of mood improvement that was hard to separate the psychological benefit of improved sexual function. So is it the chicken or the egg? We don't know, but if people have better sex, they're going to be in a better mood. And then if they are in better mode, then they will have a good sex and it's a feedback loop, right?
What is clear is that Cialis users often report better overall well-being and energy potentially because improved endothelial function means better oxygen and nutrient delivery to all organs. So we can also look at neuroprotective and neuroregenerative potential. There has been observed effects of Cials to reduce neuroinflammation and even promote neuronal growth factors in animal experiments. By raising CGMP in the brain, it can counteract the cognitive impairments in certain rotomodels of dementia. There's also interest in whether chronic cell issues might delay Parkinson's or other neurodegenerative diseases, given the enhanced blood flow and anti-inflammatory
effects to protect neurons. So it definitely goes in the realm of cognitive health for sure. Now, I thought this was pretty cool for all the performance enthusiasts out there, exercise performance enthusiast that is, about muscle preservation and physical performance. When we look at exercise capacity, As we age, microcirculation of muscles can become impaired, limiting exercise endurance and recovery. So Cialis has a vasodilatory effect extending to the skeletal muscles, which can potentially improve exercise performance.
There have even been clinical studies of patients with cardiovascular disease in which Cielis showed greater exercise tolerance. There has also been doctors that have extrapolated this to relatively healthy adults and a few small trials have tested if taking Cialis before exercise can actually increase athletic performance. And one study cited in a fitness context found that men who took Xalafel prior to a workout had higher testosterone levels after exercise compared to rise in cortisol alone in the placebo group, suggesting a more anabolic responsive training. So evidence does exist to show that again, Cials could be a performance booster.
Obviously I'm a big proponent of this. We look at the pump and recovery. So I take Cialis not all the time, but very often before my training sessions and it absolutely enhances the pumps. Bodybuilders have actually been using low dose Cials and formula is a pre-workout for years. The improved circulation during exercise leads to a stronger pump. which is not just cosmetic, it actually delivers more nutrients and growth factors to muscle fibers. So the pump isn't just about looking in the mirror when you're at the gym. It's about getting blood flow into the muscle fibres that help us. Athletes report reduced muscle soreness and faster recovery when using Cialis likely because of enhanced removal of metabolic waste and inflammation post exercise.
And unlike many over-the-counter, stimulant-heavy pre-workout supplements of which I avoid because I don't like caffeine before my workout because it can constrict blood vessels instead of open them. Cialis provides a performance boost through a healthy physiological pathway. So given its long half-life of about 17 hours, a low daily dose ensures muscles are well perfused not only during a gym session, but throughout the day, potentially aiding the continual repair and rebuilding process that keep muscles strong. We also have muscle protein synthesis. So there was a human trial at the University of Texas Medical Branch to look at Viagra, but I put this in here because of the muscle-protein synthesis,
because they both work similarly. Found that Viagar, in this case, approximately doubled the rate of skeletal muscle proteins synthesis in the fasting state compared to placebo. So the degree of the increase was comparable to the effect of moderate dose of testosterone injection, which is around 100 to 200 milligrams per week, yet PD-5 inhibition achieved it without the adverse effects noted with steroids. So I thought it was pretty interesting. Now, again, I don't think Cialis will ever replace testosterone therapy. However, in this case, it did double the rate of skeletal muscle protein synthesis.
Moreover, the men on Viagra could perform more repetitions and strength tests before fatiguing, indicating greater muscular endurance. And essentially, Cialis and Viagra seem to remodel muscle in a way that enhances both its quantity and quality. So the finding has raised the question, could long-term Cialsis use mitigate age-related muscle loss if short-team protein synthesis indicator the potential is there. And also, in doing so, This leads us into body composition, right? So there is evidence that Cialis can shift body competition in favorable ways for older adults.
So a six-month randomized trial in 2017 focused on relatively healthy middle-aged men with mild ED symptoms of lower urinary tract symptoms. And those who took daily tidalophil showed a significant increase in lean mass and a reduction in waist circumference compared to those on placebo. placebo. So the investigators hypothesized this was due to enhanced insulin secretion, estradiol reduction, which I don't think is really the case, but I'm just saying what they said, and improvement of endothelial function. In simpler terms, Cialis might help build muscle and reduce belly fat by improving metabolic health.
And supporting this, other studies have noted improved insulin sensitivity in men on daily Cials, especially those with metabolic syndrome. Better blood flow can improve the glucose uptake by the muscles acting some way like exercise does. initially by modestly raising testosterone and having an effect on estrogensialis creates a hormonal environment more conducive to maintaining muscle mass. So we're definitely going to see benefits there. And then we can look at it lastly in muscle weights and conditions. In Becker muscular dystrophy, which is a model form of Duchennes, Tadalafil was shown to rescue abnormal muscle blood flow and improve muscle oxygenation
during exercise. And so by restoring the nitric oxide pathway in dystrophic muscle, it prevented exercise-induced ischemia that contributes to muscle damage. And, so while this is a specific medical condition, It shows that improving microcirculation and muscle helps preserve muscle tissue, which makes sense, but it's cool to see how this Is being done through Cialis. So overall, Cialsis shows promise in keeping muscles much more youthful. Now, To go to the last component of aging, we have this idea of inflammation and inflammation. That's the big buzzword right now in biohacking is inflammation, so Cialis, believe it or not, and I know we're going deep down the rabbit hole here,
but it does have anti-inflammatory effects. So it's demonstrated direct anti inflammatory properties in several studies. One study knows that Tadalafel uniquely among the PD-5 drugs tested significantly lowered the release of inflammatory factors from endothelial cells and in isolated blood vessels under inflammatory stress. So this shows that Cialis's long acting nature or specific chemical structure might confer extra and extra anti-inflammatory benefit. A 2019 study on men with prostate enlargement found that those men had high levels of inflammation cytokines at baseline.
And after six months on Cialis, their inflammatory markers dropped in tandem with symptom improvement. And researchers concluded that chronic Tadalafel treatment reduced systemic inflammation, which likely helped not only their prostates, but also their cognition. So by dampening chronic inflammation Cielis could help slow the progression of age-related diseases, of which there are many. We also, in the context of inflammation, we have this improved endothelial function. Endotheliol cells produce nitric oxide to signal vessel relaxation. Cialis boosts this pathway, resulting in notably better endophelio function, even in patients who are already impaired.
A 2005 trial showed a clear improvement in flow, mediated dilation, and high-risk men on Tadalafel. Additionally, that study measured blood vessel of nitrate slash nitrite metabolites in the endothelium-1, which is a vasoconstrictor, and Tadalafel increased the former and presumably helped reduce the latter, indicating a shift towards vasodilated healthier state. What's interesting about this is the benefit persisted for weeks after stopping the therapy, suggesting Cialis might induce lasting positive changes in blood vessels and ultimately better endophelial function means more elastic arteries.
improved blood pressure control and less likelihood of blood cots of which there are numerous out there in the world today. We also have arterial stiffness and so as our arteries age they stiffen and by improving endothelial function and smooth muscle relaxation, Cialis may keep arteries more compliant. So the year-long study in older men with daily five milligram Cials saw trends toward decreased arterial stiffness, especially by 24 to 36 weeks and those at the highest cardiovascular risk. So while not every patient showed a large change, subsets did importantly, no progression of stiffness was observed in the treatment group,
which is pretty cool. Even if they didn't reverse it, it did stem the tide of it. And so even maintaining the status quo is a win when the expectations are for arteries to deteriorate. This suggests a preventative effect on vascular aging. We also have an effect on senescent cells, so everybody knows senestent cells are the zombie cells which secrete inflammatory factors in the body and then cause us harm as we age. So a publication cited by a longevity pharmacy noted that PD-5 inhibitors could relax the tension in senested cells thereby slowing aging and aging related
pathology and exerting anti-proliferative and anti inflammatory effects on cells at the cellular level. So although it's still much more theoretical, it aligns with observation that PDU-5 inhibitors are anti-perliferative, meaning they can slow the abnormal growth of cells and blood vessels, heart, lungs, and even certain cancers. So by modulating the CGM pathway, Cialis might be tweaking cellular stress responses, essentially telling overly stressed or senescent cells to calm down, which reduces their inflammatory secretions. So this area is being explored in preclinical research right now, and if validated, it would place Cialis in the category of drugs,
because it'd add as xenomorphic agents or xenolytics, modifying the behavior of aging cells. And then ultimately too, we have systemic metabolic inflammation. So some studies in diabetics in obese patients hint that Cialsis can reduce CRP, which is C-reactive protein, an inflammation marker on the body and improve insulin sensitivity, tying in to the earlier point about body composition and body-composition improvement. So by improving blood flow to metabolic organs and possibly directly affecting inflammatory signaling, Cialis could lower the chronic inflammatory burden that drives conditions like type two diabetes and atherosclerosis.
And so men with ED often have elevated markers of vascular inflammation. Obviously treating them with Cialsis not only fixes ED, but also correlates with lowered inflammatory markers and better vascular age indicators. So to sum that up, ciali seems to hit the sweet spot of reducing harmful inflammation while boosting vricular function. helps stop it. And then just to kind of close out to add a little bit more context for women, is it safe for woman? Yes, absolutely. But when we look at some of these things, women generally develop cardiovascular disease later than men, partly due to protective estrogen effects that
they have free menopause. So after menoppause, they had no estrogen or testosterone in their body anymore and Cialis can come into play. Now, like I said, you should be on hormone replacement therapy. Everyone should, but especially women after Menopausal. And Cialis has been shown to improve endothelial function, lower blood pressure, anti-inflammatory action, and it equally benefits women's hearts and blood vessels. So there is nothing that is male specific or male exclusive to nitric oxide or PD-5 in this context.
To the setting of pulmonary hypertension, Tadalafil is a standard therapy for women as well as men, significantly improving exercise ability and survival in women with that condition. As far as we know, women respond the same to men and the dosing is pretty much the It's not just about the sexual function for the sake of having sex, it's about anti-aging benefits that come from having frequently often as we age.
So like Dr. Simon, who was overseeing that study, emphasized that sexual improvements can be significant for women's quality of life. And so considering that satisfying sex is linked with better relationship, happiness and mental health and older women, as it is in older men too, this is an area where Cialis could directly enhance quality-of-life for woman. So, when we look at hormone interaction, I think that would be people's main thing. Is there evidence that it's bad? No. There's no evidence it affects estrogen levels directly, although it might increase testosterone in men. However, some researchers have posited that combining Cialis with estrogen replacement in post-menopausal women could be synergistic,
and it absolutely would. not only for sexual function, but for a host of other things. So estrogen improves endothelial function but via different receptors, Cialis could add to that effect. And also estrogen can increase nitric oxide production. Why do you think I personally inject a microdose of estrogen? Different topic for different day in a different video. But Cielis would prefer that nitroxide production, a potential one-two punch for vascular health in menopausal or post-menopause women. Also, women with PCOS, who often have high estrogen relative to progesterone and metabolic issues, might benefit from Cialis's insulin sensitizing and
anti-inflammatory effects. So I think it could be used there. Should women use it? As the standard saying goes, woman of childbearing potential should avoid Cialsis if pregnant or trying to conceive simply because of the effects on pregnancy or in study. There's no suggestion of fertility harm. In fact, some fertility doctors tried using it to improve your uterine blood flow in IVF patients. So just be aware, you may be a little bit more sensitive because usually women are smaller in nature and smaller weight wise than men. But at five milligrams daily, daily you're not going to really have any symptoms.
Just understand that your dosing might be little different than a man. I would say start with 2.5 milligrams and then kind of titrate from there. All in all, dosage doesn't seem to be that much different. And then like I said, it just comes down to psychological and social aspects. There isn't a association right now in our culture with women using Cialis. And so it's kind of one of those things that they just don't even think about it, but it could be widely used. But Cialsis is very safe and effective for women. I know tons of women that use it including my partner.
So I definitely think women should think, about using it if you haven't before. As we get closer to the end, just to sum up the dosage, It's very simple. I say 2.5 milligrams daily to start. You're going to see increased endothelial nitric oxide, decreased arterial stiffness, subjective energy and arousal boost with less than 5% of people having a headache. And then the longevity dose, you could titrate that up to five milligrams. So this is going be for most men that are over 45, women that were on hormone replacement therapy, and then anyone with BPH or metabolic syndrome.
And of course there we have the decreased risk of heart attack and stroke, decreased risks of dementia, increased in lead mass, decrease in waist circumference, and increase in muscle protein synthesis just from that five milligram dose. And I think men or women can have that. I've talked about this before of whether or not you have to cycle on or cycle off. hurt yourself by cycling off from time to time. So maybe use it for a month and take a week or two off every month or something along those lines. However, as with testosterone, I think you can do this blast and cruise kind of concept, which is meaning that you're staying on a low dose,
with just two and a half to five milligrams. And then when you want to use for super fun time, enhanced performance time you could go to 20 milligrams, so When we keep it at a low dose, which is five milligrams or below, we're getting all the benefits systemically that are happening in the body, but we are not sensitizing ourselves too much to where a 20 milligram dose is not going to do anything. Now, if you use 20 milligrams over time, your body is going adapt and it's not really going have the same effect. But if we do a load dose and then reserve the 20-milligram for one-off times where we need the performance enhancement, then we can do that.
And even if that's just as frequent as once or twice a week, We still maintain the sensitivity of the twenty milligram So do I personally cycle off? Yes. I'll take time to time off, you know, maybe one week out of every four to six weeks, I will take off. But I think if we stay on that low dose, we don't desensitize ourselves so much to the larger dose. And then we want the large dose we can take 20 milligrams and we do not have to worry about it. Basically being, having a tolerance built up to it, and then also too, We can experience all the benefits of that 20 milligram dose without some of the
side effects. Cause we have a little bit of a tolerant from the five milligrams dose Because sometimes if you've never taken Cialis and you take 20 milligrams, it's going to be way too much. You're going have a headache and then things just go bad. So what can you expect from that 20 milligram dose? Well, probably a good time. But just to sum up, Cialsis is one of the most not thought of anti-aging longevity medications. It supports cardiovascular health. obviously preserves sexual function and intimacy. It helps the brain and cognition. Helps with muscle and physical capacity and body recomposition.
And it helps with inflammation and endothelial dysfunction. So the cool thing about it is it's readily accessible, readily affordable. Pretty much anybody can get a prescription. But even if you don't want to get prescription, there are so many easy ways to find it, of which there many ways that you can it from my email list. but it so easy to fine, so cheap. and it really is one of the best medications out there all time. And it goes much further than just erectile dysfunction. So that is it for the slides.
And that is my comprehensive review of Cialis as an anti-aging tool, much more in depth than the previous year from 2024. This was 2025 style. Hopefully you took a lot away from that. I think the bigger one for me, cause I covered a lotta this in the old video, is just the women can use it. And I knew it back then, but I just had not seen it enough, I guess, from people in our community and everything that have used it, because again, it's still just not common knowledge that women could use and benefit from it but again I If you're not using hormone therapy, please get on hormone there, but you are not please at least UC Alice.
Then for people obviously on a hormone, therapy. It does a world of good to UC. Alice and I think the performance benefit aspect alone is enough for me. But then those anti-aging longevity benefits are there. And again, it's so relatively low side effect over time. is something to use very, very long term and not have any sort of issues or complications or consequences from taking a longterm as with so many other medications.
