Injectable L-Carnitine: Dosing, Pain Management, and Clinical Data
I've been using injectable L-carnitine for years, and the longer I use it, the more I love it. It's one of those staples I always keep on hand. Yes, there's post-injection pain. But the benefits are worth talking about, and I haven't done a real deep dive on this in a while.
So today we're going through the science, the clinical data, dosing, how to minimize the pain, and where injectable L-carnitine actually fits in your routine.
What L-Carnitine Actually Does
L-carnitine is a naturally occurring amino acid derivative that plays a major role in energy metabolism. Your body synthesizes it in the liver and kidneys from lysine and methionine. About 95% of it gets stored in skeletal muscle and the heart.
Its main job is transporting long-chain fatty acids into the mitochondria to be burned for energy.
Think of L-carnitine as the bus that drives long-chain fatty acids into your mitochondria. Like the Magic School Bus, but for fat metabolism. When you supplement it, you're putting more buses on the road.
Oral vs. Injectable
Oral L-carnitine has roughly 5 to 18% bioavailability. Injectable is essentially 100%. That's why injection produces much higher plasma levels than oral.
The oral form also has a downside. It can raise TMAO, which has been linked to heart disease risk at chronic high doses. Injectable largely bypasses this issue.
I prefer intramuscular over IV or subcutaneous, and I'll explain why later.
How It Works in the Body
Beyond fat transport, L-carnitine regulates energy metabolism by buffering excess acetyl groups and CoA ratios during hard exercise. This means more efficient carbohydrate use, less lactate production, preserved muscle glycogen, and delayed fatigue.
Studies show L-carnitine supplementation can lower post-exercise blood lactate and improve high-intensity work output. Same principle people associate with steroids. Steroids don't grow muscle directly. They speed recovery so you can train more. L-carnitine does something similar on a smaller scale.
It also has antioxidant effects, reduces inflammatory cytokines like TNF-alpha and IL-6 with long-term use, and activates muscle protein synthesis pathways like IGF-1, AKT, and mTOR.
The Age Factor
Like endogenous hormones and peptides, your natural L-carnitine production declines with age. Muscle carnitine levels drop. The transporters and enzymes that move it around get less efficient.
This is one reason L-carnitine has been studied for longevity and anti-aging. The older you are, the more you likely benefit from supplementing.
Clinical Data Worth Knowing
Post-heart attack mortality. A 2013 analysis found a 27% reduction in all-cause mortality after a heart attack when patients used high-dose IV L-carnitine (9 grams a day for 5 days). That's a serious number. L-carnitine is doing more than just helping you burn fat.
Dialysis patients. IV L-carnitine improved hemoglobin levels and reduced the EPO doses needed in hemodialysis patients.
Centenarians. A study on people around 100 years old gave 66 participants 2 grams of oral L-carnitine daily for 6 months. Results: average muscle gain of 3.8 kg (about 7 to 8 pounds), fat loss of 1.8 kg, reduced physical and mental fatigue, better cognitive test scores, improved walking endurance. These are 100-year-olds, not on testosterone or growth hormone.
Athletes. A 9-week trial of 2 grams a day significantly increased weightlifting volume and anaerobic power. Just 3 weeks of supplementation reduced muscle damage markers and soreness, with MRI showing half the muscle fiber damage compared to placebo.
Cognitive and mood benefits. Acetyl-L-carnitine in particular shows benefits for mild cognitive impairment and depressive symptoms in older adults, with efficacy comparable to some antidepressants and fewer side effects.
The Insulin and Carbs Question
There's a common idea that you have to co-administer L-carnitine with carbs and insulin for it to work. Some truth to this, but not the whole story.
I've experimented with both. Fasted carnitine before fasted cardio. Carnitine with carbs and insulin before resistance training. Carnitine with just carbs.
Does insulin and carbs enhance the effect? Yes. Do you need them? No.
Fasted L-carnitine before cardio still accelerates beta-oxidation and helps with fat loss. Pre-workout with carbs, you'll get an insulin spike anyway, which works in tandem with the carnitine.
I'm not recommending exogenous insulin in this video. There's a time and place, but you have to be very careful with it. Just know you don't need it to make L-carnitine work.
Is It a Fat Burner?
No, it's not a magic fat burner. But it assists fat loss.
Studies in overweight populations show modest results, around 2 to 3 pounds more weight loss over several months with small reductions in body fat percentage. Effects are more pronounced in overweight or insulin-resistant individuals.
Compared to retatrutide, tirzepatide, SS-31, or semaglutide, L-carnitine is a complement, not the main driver. Those agents do the heavy lifting. L-carnitine supports the process.
Dosing
The sweet spot for me is 400 to 500 mg injected intramuscularly. That's where the benefits become really noticeable and the pain stays manageable.
If you're smaller or a woman, start at 200 to 250 mg and see how you feel.
I've gone up to 1 gram, and honestly the post-injection pain isn't worth it for me most days. I'll do it before a workout I really want to be on for, but 500 mg is my regular dose.
For needles, a 28-gauge half-inch insulin pin works fine for most people. Anything smaller than 29 gauge and the carnitine is too viscous to push through.
The Pain Problem
This is the one real downside. L-carnitine and glutathione are the two things that just burn and sting. You can get a welt or lump that lasts 2 to 3 days.
I've had more pain from carnitine injections than from testosterone or pretty much any peptide I've used.
What helps:
- Stay at 500 mg or below per injection site
- Use a massage gun right after
- Find a buffered brand (some exist, won't name them here so the video stays up)
- Rotate injection sites
Don't go subcutaneous. People do it, but the welts and pain on your belly are usually worse than IM in your glute or delt. IM is my best practice.
Side Effects
At high oral doses (3 to 5 grams a day), you can get GI issues like cramping, vomiting, diarrhea.
There's also the fish smell thing. Carnitine metabolizes into trimethylamine, which can give off a fishy body odor at higher doses. My wife has told me about it before. People have messaged me asking about it. Yes, it's a real thing. Harmless but socially unpleasant.
TMAO concerns are mostly with chronic high-dose oral. Injectable produces much less.
There are theoretical interactions with thyroid hormone at very high doses (think 5+ grams a day for long periods), and potential interactions with anticoagulants like warfarin. At normal doses I haven't seen issues.
My take
L-carnitine is a staple, not a miracle. It won't replace hormone therapy or a serious fat loss peptide. But it complements them, and the longevity and performance data is solid enough that I think nearly everyone can benefit.
The older or more frail someone is, the more they get from it. Athletes get meaningful recovery and performance benefits. I've even seen it raise total and free testosterone in younger men with low levels.
If you can tolerate the injection pain, 500 mg IM is the sweet spot. I always keep it on hand, and when I run out, I miss it.

Hey everybody. This is Hunter Williams. I hope you're doing amazing wherever you might be in the world. Today's video is going to be all about injectable L carnitine. It's been a while since I've talked about injected well carnivine and I'm used to for years. The more and more I use it, the more more it's just always one of those things. I'm like, man, I love this. I just, love using it. It feels good. There is the downside that you do get post-injection pain. And I can talk a little bit today about how to minimize that and how the offset some of that.
But the longer I use injectable L-Carnitine, the more I am just like man this is one of my favorite things to have. and I realized in the course of talking about a lot of hormones and peptide stuff I really just don't give it that much attention. So today I want to do an update video on L carnitinine. I don't recall, I may have, because it's been years that I've been doing this, a video that actually walked through some of the human clinical trials on it. There's some human trials around IV administration, oral administration. Fewer about IM, which is what I'm going to be talking about as far as administration goes.
But I love L-Carnitine and we're going look at some the data. today around it and ultimately my experience and how to use it, and what the best practices are, because you probably will hear different things from different people, to which I'm sure most of them are probably right, but I'll give you my opinion on what to do use, how do you use that, what dose to you. How to minimize some of the post-injection pain and kind of where you might want to end up using it in your own optimization practice on yourself. So that's what we're going to talk about today. I am super excited for this one because it's been a while, but I've got more data today on ElCarnitine.

Let's get into it and today we're going to talk injectable L-Carnitine. All right, today's gonna be the injectable L-carnitine deep dive. We're gonna go to the benefits, science, practical applications of injectible carnitines so you know how to navigate the use of it.
Just to give you a bit of background so that you actually understand why we're using L carnotine as a supplement. It is a naturally occurring amino acid derivative that plays a crucial role in our energy metabolism. So when you think about carnutine, think of energy. It is synthesized in the liver endogenously and the kidneys from lysine and methionine, and it's also stored mainly in tissues with high energy demand. About 95% of it in our body is stored in skeletal muscle and heart. The primary function is to transport long chain fatty acids into the mitochondria where they can be oxidized for energy.
This makes it essential for fat metabolism and energy production. So if we think about this, I love mental models and analogies and metaphors. Think about L-carnitine as one of the buses that is getting long chain fatty acids into the mitochondria. So we supplement L carnitin. It's going to take those long-chain fatty assets and drive them on the magic school bus into them. I used to love magic schools as a kid. They would like shrink the school lesson, go into whatever not to rabbit hole, but That's essentially what L-Carnitine is doing.
We obviously have all these different forms. I am a fan of the injectable form. The oral form is good too. I'm sure it works good IV actually we'll see in some of the day-to-day it might actually not be as good as the intramuscular in my opinion but when we do that it bypasses that digestive tract there's a medical use for it so it is FDA approved for primary systemic carnitine deficiency and secondary deficiencies
in dialysis patients and then the oral bioavailability based on the data that I could gather has around five to eighteen percent bioavailability injections obviously are going to be pretty much a hundred percent bio available to get significantly higher plasma carnitine levels. And we'll talk about why that's important to why you didn't say, well, if I get 5% oral, why don't I just take 20 times the dose to the same as the injection. It will talk that while you might not want to do that today because there's kind of a downside to it. Let's look at the mechanisms of action.
Like we said, it works on fat transport and energy production. So the first thing we have is fatty acid binding. L-carnitine conjugates with fatty acids to form acyl carnitene complexes. And then we had mitochondrial transport. It shuttles those fatty assets across mitochondria membranes for oxidation. As a result of that, we've ATP generation or energy generation. Fatty acids undergo beta oxidation, generating acetyl COA and ultimately ATP energy. So when we think carnitine, it's helping produce more energy and also works in doing this with metabolic regulation.
Beyond fat transport, L-Carnitin regulates energy metabolism by buffering excess aceto groups and coenzyme A ratios during high intensity exercise. Acetyls COa or acetyl CoA buildup can inhibit glucose oxidation and L-carnitine forms acetal L carnitin freeing up Coa and for continued operation of the pyruvate dehydrogenase complex and the Krebs cycle. This leads to more efficient carbohydrate utilization so your body is more efficiently or more able to efficiently use the carbohydrates we're eating instead
of turning them into fat, reduce lactate production, meaning that we can go further without getting sore, preserved muscle glycogen and delayed fatigue during strenuous exercise, which is why I love so much injecting a pre-workout, whether that's a cardio workout or a strength workout. And studies show l-carnitine supplementation can lower post-exercise blood lactate, improve high intensity work output, which basically just means the same thing that everyone associates steroids with is steroids don't grow muscle. Well, they kind of, they allow us to recover quicker so they can work out more, thus we can build more muscle.
So we shorten the amount of time that the body needs to recovery because we are supplementing with L-Carnitine. Thus we get better recovery from our workouts and we'd get overall workouts because don't get as tired as fast. We can workout harder or you can get the same and get results. Whatever you want to do. Let's look at some of the additional protective mechanisms. So there are antioxidant properties to L-carnitine. It reduces markers of oxidative stress and muscle damage after exercise by scavenging free radicals and stabilizing cell membranes.
This also leads to anti-inflammatory effects. Long-term use actually significantly reduces pro-inflamatory cytokines like TNF-alpha and interleukin-6 that actually rise with intense exercise. And then we have muscle protection. It activates cellular pathways like IGF-1, AKT, and mTOR that enhance muscle protein synthesis and prevent protein breakdown. And, then as I've been talking about recently, we had age-related decline, just like endogenous hormones, Just like Endogenous Peptides decline.
Endogeneous L-Carnitine production declines. titular levels of L-carnitine decline with age, an expression of carnitin transporters and enzymes is lower in older adults. This age-related decline in carnotine and mitochondrial function is one reason L carnatine has been investigated in longevity and anti-aging contexts. And why I think, assuming you can tolerate it relatively decently, it's good to supplement it because our natural, not only is our national production to go down, but the enzymes and trans porters are lower.
Thus, when we supplement with it, And just looking at this, the body maintains tight homeostatic control without additional interventions like co-administering carbohydrates or insulin or prolonged dosing. Only a fraction of injected L-Carnitine may be taken up by muscle with the majority excreted via the kidneys. Now I want to talk about this and this is the reason I put it in there because there is this I won't call it a myth because There is some truth to it about administering L carnitinine with carbs and insulin. And I have done this before just to experiment for myself, meaning that I've taken l-carnitine fasted and done a fast workout,
usually a cardio session, and I done carnitin with carbohydrates and insulin to see how I respond. I've also done a resistance training workout without carbohydrates. Well, I usually always do a resistant training work out with carbohydrates, but I have done it with and without insulin to see how it is. Now, do I feel that the insulin and carbs helped the carnitine work better? Yes. However, Do I need those things to make the Carnitin work?
No. I'm of the opinion that I couldn't really find any scientific literature around this, but the carbs and insulins work great with L-carnitine but we still get a benefit without them. I think there's a benefits to taking it fasted in terms of beta oxidation when I am doing a fast at cardio session, the carnitin is going to help accelerate that which helps with fat loss. But also if I have been doing resistance training, if take it with carbohydrates, I naturally gonna have an insulin spike anyway which I want before my
workout And the carnitine is going to work in tandem with that. And so when I've exonically administered insulin, which I'm not going talk about and recommend in this video, because you have to be extremely careful with it, there's a time and place for it. We have be very careful of it and that's outside the scope of this videos. But when i co-administrated it with or without insulin was there an enhanced effect? Yeah, sure. I would say like I felt more of a pump and felt endurance and everything. But also, administering with insulin, using carbs and insulin is kind of going to help drive that anyway.
And you can do insulin pre and post or just post workout. There's different schools of thought around that. I've done them all. You can even do an intro workout if you want to. But there is a performance enhancement benefit to using it with Insulin, but it is not necessary. So if don't want, you have to go and source insulin and figure out how to dose insulin. It's not dangerous per se, but you do have to be careful with it, more so than you would like a PPC 157 or even carnitine.
There's a time and place for it. But I just wanted to kind of put that out there that in order for the L-Carnitin to work, pre-workout, you don't have have insulin with, it but it can help. So there you have it with that. I did want to address that end here. When we look at the medical applications of it, obviously we're talking about using this for performance and longevity purposes, but when we looked at it in the medial world, it was using the dialysis patients, they used IV carnitine to improve anemia and muscle symptoms in hemodialysis patient.
Meta-analysis showed significantly increased hemoglobin levels and reduced erythropoietin doses needed when they were using IV-carnitin in dialyses patients was pretty cool. We look at cardiovascular support, high dose IBL carnitine. What they did was nine grams a day for five days after acute heart attacks improved outcomes, reducing ventricular arrhythmias and angina frequency. A 2013 analysis found a 27% reduction in all cause post myocardial infarction.
when they used L-carnitine. How about that? We were talking about this obviously in the context of performance, right? In longevity and fat loss and all that stuff, which is great. But how about a 27% reduction in all cars mortality post myocardial infarction? Pretty interesting. And it goes to show that L carnitin is doing a lot more things than just helping us burn fat or get a better pump at the gym. It's making us healthier. Obviously this is a high dose, nine grams. of IV L-Carnitine. So that's a pretty high dose, right?
If you could inject 9 grams intermuscularly, you have glutes or delts of steel. Let's look at some clinical data around athletic performance. Research on injectable L carnitinine in healthy individuals and athletes has revealed a crucial finding. Simply elevating blood L Carnitidine with IV infusion does not substantially increase muscle carnituine levels on its own. IV infusion alone maintained 10-fold elevated plasma carnitine for five hours, showed no change in muscle carnatine content.
And when adding high dose insulin, which I do not recommend, when having high-dose insulin accompanied carnotine infusing, muscle carnotine rose by about 15%. And also, too, we look at loading of the oral version after 12 weeks of oral L-carnitin at two to four grams per day with carbohydrate drinks. Muscle carnutine increased around 10% with improved performance. So we don't have any published exercise trials explicitly comparing IM or IVL carnitine or oral in athletes under identical conditions. Evidence suggests injectable carnotine can acutely raise blood levels for far above oral supplementation.
Now let's look at the medical dosing for deficiencies. When we talk about the oral dose, most people are going to do 50 milligrams per kilogram per day, which I think comes around like three grams for the average person, three to four grams, for a 70 kilogram person with deficiency. If you were doing the oil version, that's what I would recommend. However, we do have this thing called TMAO and When you take oral l-carnitine, there is gonna be the side effect of potentially raising TMAO, which could be down the road, harmful to the liver.
Now, I think you're gonna have to do high doses for long periods of time to that, and I three to four grams orally is probably safe. However, when we talk about the injected versions, we bypass that altogether. But injectable is very effective for quickly correcting deficiencies. And an inherited primary carnitine deficiency, IM or IV, L-carnitin rapidly restores normal levels and prevents organ damage that could come as a result, potentially, from carnotine deficiency.
It's very well safe and tolerated clinically. There was this pretty cool, again, this is oral, but there was a pretty study I found on centenarians, which were people that are around 100 years old or older. And they had 66 of them, and they were given two grams of L-carnitine daily for six months. And it showed significant improvements compared to placebo. I think when we look at carnitin, maybe a 19-year-old doesn't benefit as much from supplementation, but someone that's 90 or 100 years old benefits greatly.
Interestingly enough, obviously these people are a hundred years old. They're probably not changing that much of what they're doing. If I had imagined, could imagine the study, they had 3.8 kilos of muscle put on, which I guess that would be somewhere around 3 .8 times 2.2, somewhere in the neighborhood of like seven to eight pounds of muscles. And they also had 1.80 kilos fat loss. So the average muscle increase was 3 point 8 kilos. The average fat lost was 1 .80 It's kind of crazy considering these people most likely that to my knowledge, weren't using testosterone or growth hormone
or anything. But I think it goes to show, are you going to do that if you're 45 years old? Probably not. If you've taken two grams of oral carnitine day. However, when we look at what it's doing to biological systems in the body, there's clearly a benefit in people that are deficient. When we looked at the benefits, they had reduced physical and mental fatigue, improved performance on mental function tests in this study, increased walking endurance, better ability to perform data activities, and no serious side effects reported. And so who benefits the most?
The frailer or more carnitine deficient individual, the more pronounced the benefits. And I think when you look at people who are a hundred years old, obviously like we talked about the enzymes, natural endogenous production, are most likely not going to be what they once were. very old or frail individuals with low muscle carnitine showed increased muscle mass and exercise tolerance while healthier, older adults may see smaller effects, but effects nonetheless, I would say. And from a longevity perspective, indirect benefits include improved mitochondrial health and better metabolic markers. Animal studies show carnotine rejuvenates mitochondria function and reduces age-related oxidative damage when combined with alpha-lipoic acid,
which I think is a good supplement to take as well. Let's look at athletic performance and recovery. There was a nine-week trial of athletes that did two grams a day of L-carnitine, significantly increased weightlifting volume and anaerobic power. By week nine, higher average and peak power on wing gate tests with lower post-exercise lactate levels like we talked about. Just three weeks of L-Carnitine at two grams a day reduced muscle damage, markers and soreness. MRI scans showed half the muscle fiber damage controlled or excuse me, compared to placebo after workouts.
It's pretty cool. Again, this is oral. Imagine if you're doing the injectable, how much higher that's raising muscle carnitin levels. Then after 24 weeks of carnitine plus carbohydrate administration, loading muscle carnatine by around 20%, trained subjects experienced longer at 80% VO2 max, used more fat, less glycogen, and reported lower perceived exertion, which is always nice when we are working out hard, but we don't feel like we're working hard. In the early 1990s, research found no benefit with short-term supplementation due to insufficient dosing duration and lack of muscle uptake.
The modern evidence-based approach requires either pairing carnitine with insulin spikes or using it chronically, to which I would just say, sure, yeah, does it help? You don't have to. But when we look at it, if we are trying to maximize muscle creatine, stores, that's what we do. I think it's advantageous for all athletes. Is it going to make or break someone? No, like is it gonna make you shoot a basketball better or run a football faster? no, but it is a supportive aid, much more so than people give it credit for, in my opinion.
When we look at general well-being and metabolic health, we have this fatigue reduction that I think is important for everybody. It combats chronic fatigue by bolstering mitochondrial energy production. Studies in chronic fatigued syndrome cancer patients and healthier older adults showed improved symptoms and increased vitality. Like we talked about, the centenarians reported decreased physical and mental fatigue. enabling easier performance of data activities. Also cognitive function. I would say the injectable does produce, at least in my case, a slightly noticeable cognitive benefit.
The days that I do that, I feel good. Feel good most days, but there is. However, if you wanted to maximize, there's a form called acetyl-l carnitine. This was what I was talking about. Usually will take two 500 milligram capsules of this, which is 1000 milligrams, one gram. of acetyl-L-carnitine first thing in the morning. And there's, there are benefits to show that that helps with cognitive health trials and mild cognitive impairment report, slower decline or improved memory meta-analysis of a acylo-l carnitin found it effective in reducing depressive symptoms,
especially in older adults with efficacy comparable to some antidepressants, but fewer side effects. Again, this is one of those things we have the depression and anxiety conversation. How much of that is related to someone's health that could be improved with something as simple as supplementing with L-carnitine? I think it's one of those things, it for some people, depending on where they're at, can almost be like a wonder supplement, you know? But we look at metabolic health, L-carnitine aids insulin sensitivity and our lipid profiles.
Trials in type two diabetics show improvements in glucose tolerance and insulin resistance. And by clearing fatty acids from muscle cells, carnitin reduces lipids-related insulin resistant, which most, I would say, lipide- related insulin resistances, is lipid issues are driven by insulin resistance in a lot of cases. And some studies note modest reductions in fasting glucose and A1C. Also, like we talked about, it helps with heart health, is used as a complimentary therapy for angina and heart failure.
It can improve exercise tolerance in those with chest pain and is shown reduction in symptoms or hospitalization and hard failure trials. Some dialysis patients see increased HDL cholesterol, and post heart attack patients experience reductions and triglycerides and total cholesterol. Let's look at it. Obviously, everyone wants a fat burner, right? Is this real or is it overhyped? Ultimately, I'm going to say no, it's not a magic fat burner, but it can assist fat loss efforts. It can work really well with other peptides.
In the early nineties, we had short term studies that found no significant effect on fat oxidation or body fat because muscle carotene levels didn't change. Recent research in overweight population shows modest benefits and it led to two to three pounds more weight loss over several months with small reductions in body, fat percentage. And the effects are more pronounced in those who are overweight or insulin resistant. And it works best obviously when combined with diet. We obviously have that centenarian effect and then indirect benefits and younger adults carnitine might indirectly aid fat loss by boosting workout performance
or enabling higher training volume. It may also reduce cravings and improve energy for an active lifestyle by alleviating fatigue. Ultimately, L-Carnitin is not going to be this like magic in my opinion. Now, if you're scaling the dosing to like five grams a day, I think it would probably be non-linear, meaning that the results would be more than like upping from 500 to a thousand. If you go from a 1,000 to 5, 000 milligrams injected, yeah, you're probably going to notice better muscle gain and fat loss benefits because of what it's doing. However, I think relative to what we have that is easily administered and we relatively easy access to Reddit True Tide,
Matzi, SS31, SLU some of these things out there that are so potent for fat loss. Oh, carnitine is a compliment to them. I would not say it's going to do. the, the I was going to say damage, but I'm gonna say do the do, like heavy lifting that those agents will do. But it works extremely well as a compliment because of this whole beta oxidation thing that is going on in the body. It's not a magic fat burner, But, it will help. it is better than not using it. If we look at side effects and safety, I get asked a lot about this.
I wanted to put it in here. Obviously if you do really high doses, especially of the oral, It can definitely have GI side-effects, cramps, vomiting, diarrhea. More common, and usually you're going to see that around like three, four, or five grams per day in my experience. I mean, I can take up to three or four grams and don't notice it, but depending on how big someone is, it can affect them. This is a real thing. If you get injectable carnitine and you smell something that smells like the Wuhan fish market, that's just par for the course.
It is not something wrong with it. And also too, it can actually have the effect of making you have a slightly fishy odor. I talked to my wife, she's very familiar with me having lots of body odors emanate from me because I am someone that sweats a lot. And so it is a real thing. And, so people have sent me messages before like, hey, my wife noticed I smell like fish. Is this from something I'm doing? I was like taking out carnitine and the answer was yes. It's pretty infrequent, but basically what happens is that carnotine metabolizes into trimethylamine.
and it usually only at higher doses, again, over a three grams a day, harmless, But obviously that is socially unpleasant if you smell fish, So there you go. We have injection side reactions. Obviously this is the most common thing, To me, going over one gram of injected Delcarnitine per day, although the benefits are numerous and incredible, there's this downside and it ends up being that I have a welt on my butt or my delt where I injected it and is painful for days to come.
I have had more pain from carnitine injections than I ever have from testosterone injections and pretty much any other peptide injection. If there's one thing, well, there are two things, carnotine and glutathione. They burn and they sting. And a lot of times you're going to get a lump from them. Now there're things that you can do. There're are things you could try to buffer it with, whatever. But at the end of the day, it is what it it. The higher the dose, usually the worse the pain is. For 200 milligrams, a lot of times it's not that bad.
If I do 400 to 500 milligrams which is my typical dose, usually I get away with it being okay, but still it it literally a coin flip over whether I post injection pain or not and usually it takes two to three days to go away. Yes, you can use a massage gun right after and that helps. yes, You can get massages done. There are brands that make ones that are buffered that don't hurt as bad but I've still used those and although better, still not the best. I talked about this TMAO. High levels have been associated with increased heart disease risk.
Chronic large doses of oral L-carnitine can elevate Tmao. Injectable L carnitin might produce a lot less TmAo, but it's still consideration if you're doing high doses. So just be careful. I've never heard of anyone running into an issue with that from L Carnitene, But you would probably have to be doing like 10 grams a day for several months, if not years at a time. It also could potentiate anticoagulant drugs like warfarin. Now, I don't know how common that is, but I did see that. And it can also interfere with thyroid hormone activity. High doses inhibit thyroid hormones from entering cells.
I said reports of triggering seizures in those with seizure disorders as well. In my experience, doing a dose of like 90 milligrams or 60 milligrams desiccated thyroid, not an issue, If you're doing like super high doses of L carnitine, I think it potentially is one of those things that are going to be an interaction with the thyroid. And I, think if you were doing a long-term super-high doses, not 500 to a thousand milligrams, but like five grams per day, maybe over time we see some thyroid dysregulation. Although I have not seen that in practice from anyone that I've talked to, it is a mechanistic thing that you would have to acknowledge is possible.
When we look at the dosing, I think 200 to 600 milligrams per day is the sweet spot. If you're smaller or a woman, start lower, 200, 250, see how you feel. I end up doing 500 milligrams. Honestly, love doing a thousand milligrams, but honestly, just can't get it to feel good after the injection. And so if I've got a workout that I really want to be on the ball for, i'll do that. But you can do it. 500 milligrams, and to me, I get like really, really good benefits.
So start conservative, 200 milligrams. Tighter it up. I think most people will find their sweet spot, man or woman, regardless of size, 500 kilograms. That is kind of the sweet The pain isn't too bad. It's bearable. And then you also to get a prenotable benefit, whereas like 200 milligrams, better than nothing. Yeah, sure. But it's not, it, 500 milligrams is definitely better. I'll say that. Then, you know, just keep it consistent.
resistance training. The one thing I will say is a lot of people think you have to use like a 25 gauge. I have always been fine. Obviously this can kind of depend on the level of body fat you. Have I've always. Been fine with a 28 gauge half inch. And that's actually considered an insulin needle. So a 20 gauge, half-inch probably okay with the 29 inch, anything smaller or excuse me, 29, I hope it's not 29. It's 29 gauge have inch anything. Smaller than a 29 gage, like 30 31 gauge It's not going to push through. It was too viscous. it's a bigger molecule. So it was just not gonna push those. You really need that 28 gauge.
And obviously we're doing this intramuscularly. I did want to address subcutaneous administration. you can absolutely do subcu administration, here's the thing though that's going happen, you're probably going get a well and it is probably gonna hurt. Do it, be my guest, do it. i'm just a fan of it though. People say well yeah you are going a get well with IM. Its much less likely that you will. And I will tell you, the most pain I've had from carnitine injections, although if I'd done like two grams, that does hurt and leaves a well. But when I done sub-Q, it does not feel good.
And sometimes having a lump on your belly is more painful than having lump in your glute or on you delt, and I'll say that much. It's up to you at the end of the day, but that is my best practice. I know people do it and they're fine doing it, At the end of the day, it's a valuable tool. It's not a miracle, but it continues to be one of those things I just always have. I always had it when I run out. And when don't have it, I miss it. There's obviously a lot of evidence-based benefits when we look at it from a longevity or a performance perspective. So it was one those thing that everyone can benefit from. your 16 year old son playing football, trying to go to college, I wish I would have had injectable carnitine because that would've helped me.
I think there's also, and I've talked about this before, that I have seen it raise total and free testosterone levels in younger men that have lower levels than they probably should. There's a benefit there. It's one of those things, again, it's the staple, It is not a miracle, its not as powerful as something like a retitrutide or even hormone therapy, but it is good to have and if you can stomach the injections, it works well. At the end of the day, I think the best dose is somewhere 400 or 500 milligrams and injected intramuscularly. And that is it for the slides. That is my most recent deep dive update on L-Carnitine.
Hopefully it was helpful, educational, and informative at the same time and maybe even a little entertaining in there. Again, i'm a huge fan of it. Like I said, always keep some on hand and keep it with me. The only downside to it again at the end of the day is you do sometimes get lumps. There are brands out there that you can find that I'm not going to mention the video because I would like the videos to stay up. But there are other brands that have buffers in them that help with that and make it more tolerable but it's still going be one of those things I'd say.
At the end of the day, L-Carnitine and Glutathione will still be one of those ones that they're just going to burn. And they are actually ironically, or unfortunately, two of most beneficial things that we can do just for anti-aging, longevity, athletic performance support. I love L-Carnitine. I think everyone should be using it. If you're not on it, there's so many peptides out there that are amazing, but I would say have more of like a specific use case. L carnitin is absolutely one of those ones. Young man, excuse me, young old man woman, whoever can benefit from the administration of L Carnitene.
And it can really improve their life. We look at weaker or more frail people tend to benefit more from it but even someone that wants or needs to lose a ton of weight can still benefit it from too.
